Summer Camp Registration Instructions

Registering for Freestyle America Summer Camps has never been easier – It’s as easy as A – B – C. Please follow the instructions below to guarantee your space at camp.

FILL OUT ENROLLMENT FORM AND WAIVER

Download the form by clicking the link above.
Fill the form out completely, making sure to indicate which camps you are signing up for. If you are signing up more than one athlete, please fill out a form for EACH athlete.
ENCLOSE A PHOTOCOPY OF YOUR HEALTH INSURANCE CARD (FRONT AND BACK).
Mail the forms and copy of health insurance card back to Freestyle America with your check to the mailing address above.

SEND PAYMENT IN FULL

In order to guarantee your spot at camp, you MUST pay in full.
All fees must be paid in US Currency via money order or check.

BECOME A USSA MEMBER

Freestyle America is a USSA ski club. You MUST be a current USSA member to participate in camps. Memberships expire annually on June 30.
For a 08-09 membership, simply watch www.ussa.org in early MAY for online renewal and follow the instructions. Fill in all required fields. Online memberships result in IMMEDIATE renewals.
If you must send a hard copy, please fax it (435.647.2052) with your camp dates clearly noted. Allow for 3 weeks to process hard copy memberships.
MAKE SURE that you have a license that is current DURING the camp you are attending. See Whistler Special Notes.

Once we have received and processed your payment, we will mail you a Welcome Packet. In this packet, you will find a letter from the Directors, a list of things to bring to camp, and the appropriate paperwork that you will need to fill out for each athlete, which will need to be mailed back to Freestyle America.

We are honored to have your youngster at camp with us and are absolutely committed to his/her positive experience! PLEASE HELP US spend more time with your child and less time processing last minute paperwork by returning forms and payments THREE WEEKS PRIOR TO CAMP! Incomplete registrations are subject to a $25 handling fee within two weeks of arrival.

PLEASE NOTE: Camp Registrations are WELCOME, providing there is space, until the first day of camp. Join us for lots of fun in three beautiful locations!

SPECIAL NOTES ON WHISTLER

All camp participants are required to have a US passport.
All camp participants need both a 07-08 license AND a 08-09 USSA license. Make sure you complete the process listed on the left AND follow up with a phone call to USSA. Athletes without current USSA licenses are NOT allowed to attend.

WHISTLER CANCELLATION POLICY

Until May 1, 2008 you can cancel less a $150 handling fee – after that, all camp fees are non-refundable for any reason.
We strongly recommend you purchase trip insurance to cover non-refundable costs in the event you need to cancel for injury, etc.

TRAMP CAMP / RAMP CAMP CANCELLATION POLICY

For all camps, applications are welcome up to the first day of camp.
Cancellations PRIOR to 30 days of arrival will receive a full refund less a service fee of $75.
Cancellations WITHIN 30 days of arrival will not receive a refund.
